Default Populating comments to a word document

A buddy sent me a questionaire (word document) with 20 or so questions,
behind the exclaimation mark of each question, there were gray boxes for me
to type my responses. Some of the boxes even had drop down list (if it was
day of the week question). I could type my responses in the boxes and the
underlines below the boxes were not altered. It was really neat. Is someone
familiar with what this feature and those boxes are called? How do I access
it?

It's called a Form. Follow the link below and you will learn all you ever
wanted to know about forms and then some!
