Default Locking checkbox and text fields after it's filled in

I created a form which has checkboxes and text fields that will need to be
filled in. Once it's filled in, I would like to know if there's a way to lock
that person's selections/comments because after that person fills it out, it
will be sent to someone else to comment.

I have Word 2004 MAC version.

What do you mean by "locking"? A protected form *is* locked, in the sense
that you can only change the fields.
