How can I place several form fields in a sentance without the text moving to
the right and changing the appearence of the entire document

example: terminating at the _____ day of _____, 20__ or any renewal thereof.

In running text of this nature, the whole idea of form fields is to mesh
with the text and create an appearance of continuity (just the same as would
happen if you typed the words into a document). The fact that a document is
a form should actually *not* be obvious, especially in legal documents. It
is far better to give the impression that the document has been created
expressly for the purpose.

Use real form fields, not underlined spaces.
