Is there a way to place a shadowed checkbox in a form?

If you mean in a form field, then no. What are you trying to achieve?

Hi, Graham:

Our Word Processing Department has been asked to place a form onto our
system with the request that it be exactly like the one presented. The check
boxes in the form have shadows; other than that the form looks like something
I can do.

At the moment I'm waiting to see if there are copyright issues, which might
make this a moot question. But just in case we are able to put it on the
system, I'm concerned about those check boxes.

You might be able to cobble something by using small dark coloured text
boxes behind the fields.

Graham, I was "saved by the bell." Turns out there are copyright issues, but
I intend to try your "cobble" suggestion just in case it comes up in the
future.
