Default creating fillable lines

I am trying to create a general notice form with fillable lines so when
information is typed on the line or changed, the line itself will remain the
same without being affected.

Any help is greatly appreciated.

the forms tutorials by Dian Chapman that this article links to. Note that,
if the forms will be filled out *only* online, the lines are quite
unnecessary.
