Default Word Document Form

Can anyone help? I am looking to create a word document
form that enables the user to "freeze" a certain portion
of a line so that this form may be used as a template
to "fill in" information. For example, line 1 would
include Last name:___________ First Name:___________ If
the lines could be frozen, characters could then be added
in and the filled in characters would not shift the line
to the right, etc. Please provide assistance. Thanks.

See http://word.mvps.org/FAQs/Customizat...nTheBlanks.htm and
especially the forms tutorials by Dian Chapman that this article links to.
The usual way of formatting forms is using a borderless table with bottom
borders where lines are desired.

What you are talking about is what Word calls an "online form." Check this
in help. For more about online forms, follow the links at
http://addbalance.com/word/wordwebresources.htm#Forms or
http://word.mvps.org/FAQs/Customizat...nTheBlanks.htm especially Dian
Chapman's series of articles.

Hope this helps,
