Default Setting up an index directory in a Word doc does NOT work / "No index entries found"

Ok, I have a Word 2003 doc which contains currently NO index directory.

To setup such a index directory I did the following (according to some MS manuals):

I ...

1.) Marked a line/words
2.) right clicked and selected "Bullets and Numbering"
3.) Selected "Outline Numbering" and "1.1.1" style
4.) Repeated 1. - 3. for approx. 5 other lines

5.) Set the cursor at the top of the document
6.) Clicked the Word2003 menü "Insert-Reference-Index and Tables..."
7.) Clicked OK

Now at the place where the index directory should appear an error line was shown:

"Error! No index entries found."

So what ? I already marked a couple of text phrases.

What else can I do ?

Perhaps "Bullet and Numbering.." is not the corrent index entry definition.
But what else is correct?

When you say marked an entry... did you go to Insert, Reference, Index and
Tables, Mark Entry, Add Main/sub entry clicked mark/mark all for each word /
phrase you want in your index?
