I was using a word form and I noticed it went blank and I could not get my

Chances are that you cannot. Going blank usually indicates that the fields
were reset.
If you close the form without saving it, your previous saved version may
have your data. What you are talking about is what Word calls an "online
form." Check this in help. For more about online forms, follow the links at
