I have a document with many hypertext links. When the cursor hovers one of
these links, the user is prompted to 'control click to follow' the link.
What is the key stroke combo to return from the hypertext link?

It can depend on what the link links to, but generally speaking, use
Alt+Left arrow.

A control left click on an item in the index sends focus to a place that Alt
left arrow does not return from. What works for that?

There's no single solution to this. Alt+Left Arrow normally works; if not,
try Shift+F5.

If neither Alt+Left nor Shift+F5 work, try Alt+Tab. Alas, there is no one
solution.
