Default Initial zeros in zip codes getting dropped in Word mail merges

I'm doing mail merges in Word, using an Excel spreadsheet for the mailing
list. The zip code column in the spreadsheet is defined as zip code data and
the cells display the initial zero correctly (e.g., Natick, Mass. is 01760).
But in the mail merges, the zero gets dropped and the return address on the
letter prints as 1760. How can I make the initial zero print?

Along similar lines, is there any way to put two spaces between the state
and zip fields? The merge function only puts one space between them.
