How do I create a form to set up multiple choice circles, where only one
answer can be chosen?

How do I create a form to set up multiple choice circles, where only one
answer can be chosen?

The Controls Toolbox has a "radio button" you can insert on your document.

If you need more than one group of mutually exclusive choices, right-click
the buttons, choose "Properties" and give each group its own NAME.
