Default Form Template

I need to create a form that has standardized questions and allows for
answers to be typed in by person answering questions.

It is to be used as a telephone infomation gathering form to get information
from general dentist offices that call in patient referrals to a specialty
dental practice.

Upon completion, the form will need to be e-mailed to the Marketing
Personnel to trigger a follow up visit to the referring dentist.

What you are talking about is what Word calls an "online form." Check this
