Default how do i modify normal.dot to open with header/footer visible

I want to always open every new document with the normal.dot template and
have it show header and footer boxes. How is that accomplished?

Open the normal.dot file (located in the template directory), have the header
/ footer boxes showing and then save it.

You will need to (a) be in Print Layout view and (b) have "White space"
checked on the View tab of Tools | Options. See
