Default Form Fields lose bold formatting

I received the following from a user today, but was unable to find anything
in the knowledge base and didn't see this addressed here.

In working with a customer this morning, it became apparent that sometimes
form fields in Word lose their bold formatting at the time of fill-in.

I've attached the table this came up in. What I discovered is that if I
remove the number formatting, then bold sticks. Otherwise, sometimes bold
sticks and sometimes it doesn't in the fields with number formatting.

On the attached table, I've changed the formatting to regular text in all
but the last 2 fields. The last field has dollars and cents formatting and
the next to last has whole number formatting. If you tab through this a
couple of times, I think you'll see where the bold formatting gets dropped
from either or both of the last 2 fields.

I did work with this form and was able to reproduce the behavior that was
described. Any information on this would be appreciated.
