Some of the numbers of numbered heading styles are not showing up in the
Document Map. There is no rhyme nor reason to it that I can tell. The
Document Map is going to be a handy tool for a certain document I am setting
up - I really need for it to behave.

And in addition to the bugs mentioned in the article, Document Map =
doesn't show the numbering of any heading that follows a manual page =
break.
It's an old bug that doesn't seem to get fixed.

Thanks! It looks like the hard page break was my problem, all right. And it
is good to know what to do when that tiny print shows up - I have had that
happen and it drove me nuts!
