Default format footnote to no-column header followed by column

I want a header (no column), followed by columns, then a footnote to the
header, which will appear with no column formatting. I tried making an
endnote, but the results were the same as a footnote - both were formatted as
columns.

The header can be placed in a floating text box or frame that
spans the page. For the footnotes, see

See also http://word.mvps.org/FAQs/Formatting...panColumns.htm, but
if the entire document has only two columns and you want the heading to span
both, you need only format it as single column (there will be a section
break between the heading and the columns).
