I have entered all the information correctly when going through the Wizard
but when the greeting line prints is does not print like it shows in the
Wizard.

Example: In the Wizard it shows it will print "Dear Mr. Randall" but when
it actually prints, it prints "Dear John Randall".

What am I doing wrong?

Do you have a field in your data source that contains just the last name?

I assume that you are using the AddressBlock and other pre-configured merge
fields in your main document. Instead of doing that, just insert the
individual merge fields from the data source in the configuration that you
want them.
