Default Underlined Text, UnderlinedTabs, and "pseudo" form docs

I've been asked to re-create a hard-copy form in Word to appear exactly as
the hard-copy version does. Users can then use the Word version as a pseudo
form doc. However, because the formatting must remain exactly as the
hard-copy form (underlines, spacing, etc.), I cannot use Form fields.

I am having trouble keeping text underlined when typing in underlined tab
areas: when an underlined tab comprises an entire line -- i.e., is preceded
and succeeded by a Paragraph mark -- then added text remains underlined.
However, if the underlined tab comprises a portion of an entire line -- that
is, preceded by text and succeeded by either text or a paragraph mark -- then
the underlining does not remain. Attempts to set Underline do not "stick."

Possibly of note: the underlined blanks must use a different typeface than
the rest of the doc.

Any suggestions?

Thanks,

-Hans
