Default 2003 Word User Form VBE controls toolbox CommandButton

The CommandButton has disappeared. how do I get it back? I was trying to
create a userform using Mr. Robbins instructions. I went into VBE. I selected
UserForm from Insert Menu and Controls toolbox doesn't have CommandButton
Control.

I'm not sure how it's gone missing -- you can deliberately remove it, but
you'd remember doing that.

Anyway, right-click anywhere in the toolbox and select Additional Controls.
In the dialog, scroll down the list to "Microsoft Forms 2.0 CommandButton"
and check the box next to it. Click OK, and it should show up in the toolbox
as a small rectangle.
