Default Word 2002

I want to add some boxes to a document so that, when printed, it offers
either a place to put a check mark or to input a series of numbers, ie credit
card number. Any suggestions?

Although there are symbol characters that create a box, my favorite way to
do this is with the EQ \x (box) field. For more, see
