Default How do I convert chapter endnotes into book endnotes?

My client wrote a book. Each chapter is a separate document with endnotes at
end of each chapter. Publisher wants all endnotes at end of book, not end of
each chapter. This was not set up as a master document. Is there a way to
move chapter endnotes to a separate document for all endnotes, keeping the
automatically numbered endnote numbers within the individual chapters? Would
making this a "master document" be of benefit? Any suggestions?

I use Word 2003.

The general procedure is as follows:

1. Set endnote numbering for each chapter to "restart each section."

2. Compile all the individual chapter files into a single document,
separated by section breaks.

3. For each chapter/section before the one where you want the endnotes to
appear, check the box for "Suppress endnotes" on the Layout tab of Page
Setup.

Note that this works correctly only if each chapter is a single section. If
you've had to insert section breaks to change number of columns or page
orientation within a chapter, then there is no way to have continuous
endnote numbering throughout the chapter; the only alternative is to number
the endnotes continuously throughout the book.
