Default Continuation form- Word 2003

Good morning all, I want to create a form that has data at the top (name,
date etc.), data at the bottom (sig blocks with dates etc.), and rows in the
middle for multiple entries (1 entry per row). Here's the question: How do
I create the form, so that when the user puts in more row items than 1 page
can allow, the form will automatically create a continuation sheet with the
top and bottom data from page 1 on the continuation sheet?

Jim:

It sounds like you should look into Headers and Footers. These can hold the
top and bottom information you want, and will appear on every page of the
document.

In general, you only have to enter the information once, and it appears the
same on every page.

Thanks for the suggestion Bear, but because this is a user form, ISO
requirements mean that it has to be a template that is non-editable (except
for the intended fields) by the user. In a protected template, the user
cannot edit the header/footer to fill in the fields that I design.

Any other ideas? Macro maybe?

Jim:

Make the first page header and footer blank and position your control blocks
where you will. Replicate the blocks inside the second and subsequent page
header and footer and populate them using REF fields that point at the
bookmarks for the formfields on the first page.

Default Continuation form- Word 2003

REF fields in header/footer don't update automatically in protected
documents, and F9 is unavailable. Short of a macro, StyleRef fields are the
simplest solution; see http://sbarnhill.mvps.org/WordFAQs/StyleRef.htm.
