Default Index

Thanks to CybrTaz on 9/14 I now get the Index in the right place, but when I
'Insert Index' I get a ready-made one. It does reference the document but in
a useless way. How can I purge the document of this embedded index and all
its XE references in the text and start afresh? The instructions in 'Help'
don't work.
