I am creating a Survey using word and I want to insert boxes so that the
reader can place a "X" or a check in it. I would appreciate any assistance.

I have insert the check boxes using the Forms options. Next I want to know
how allow the reader to only choose one box per statement. For instance, for
one statement there are 5 choices but I want to only allow to select one or
the other. Their last selection will be the final one, answering the
previous check from another box.
