Default TOC

I am using Word 2003
I have a 3 level TOC. I want ............................ as the leader to
the page number for all 3 levels. I indicated this on the Index & Table box.
But the .............. are only appearing for the first 2 levels and not for
the 3rd level. Question #2: When I go into the Table of Contents Options
box, and indicated (checked) which styles I want for the TOC - it adds to or
changes the styles to Heading 1 through 3, which is not what I want. While
the TOC appears to come up OK, it bothers me that the Heading styles are
checked also. Can anyone help me with these two problems please???

Heading styles have built-in outline levels. Unless you clear the check box
for "Outline levels" OR remove the numbers from beside the heading styles,
the built-in headings (along with Title and Subtitle) will always be
included.

The leader problem could be caused by a misplaced tab stop, in which case
you'll need to correct that issue. But sometimes the styles do just lose
their leaders, and the only way to fix them is to go to Format | Tabs and
reapply the leader; since TOC styles update automatically, you can do this
in just one entry at each affected level, and the style will be modified and
propagated to all the rest.
