Lost quick parts on template

I have created a template with a number of quick parts - tables, headers,
footers.

I have been having some inconsistent results when I save the template as a
new version - sometimes the quick parts are lost.

Also when I attach the template to an existing document, the quick parts are
gone.

Any help please?
