Default Outline numbered lists

I have created a custom template in styles for my outline numbered list
following the directions in Shauna Kelly's article to the letter and only my
level 1 progresses to 2,3 etc. Level 2 and beyond just stay at 1.1 or 1.1.1
and won't progress to 1.2 etc. I also no longer have the option on the drop
down to change them to Level 1 instead of heading 1 as its no longer there.
How do I rectify this?

Did you make sure that the "Restart numbering after Level x" boxes are
checked for lower levels? Did you make sure that all the styles are part of
the same outline-numbered list (that is, you linked each style to the
appropriate outline numbering level)?
