Default Fax not on List of Document Types

I am using word 2003 and microsoft fax services. What do I need to set so
that faxes will be an option on the 'select document type' when I go to
create a mail merge document.

I attempted the instructions from Milly (posting 7/26/2005) without success-
outlook said that the fax number wasn't recognized.

Please Help!
