Default TOC results not consistent

Ran a Word 2002 TOC with the built-in Heading 1 and 2 styles. The result is
a TOC which lists all Heading 1 entries correctly. However, the Heading 2
styles randomly either show the numbering or don't show the numbering, in
this case, either randomly shows or doesn't show the Section 1.1, Section 2.2
or Section 3.3 numbering, but in every case shows the text after the
numbering. What can I do to show all Section numbering correctly?
