Default Customized style button stops working in Word 2000, 2002, 2003

This has been a problem in versions Word 2002, 2002, and 2003. A custom
button on a custom toolbar stops working in the middle of a Word session. It
seems intermittent. It's not always the same button.

The button was created by dragging the style from the Commands list (Tools
Customize... Commands Styles) to the toolbar.

It doesn't seem to matter if it is a paragraph or character style. I am not
using any of the normal.dot styles - this is a separate template. The base
style was not deleted or ren-named. The name of the button shows the correct
style. When you press it, nothing happens. Any ideas other than re-creating
the button?
