Default troubleshooting indexes

Some of my index entres are little tiny letters and other are regular size.
What caused this to happen?

Default troubleshooting indexes

Are they at different levels, that is, entries vs. subentries? If so, check
the formatting of the corresponding Index style to see if it is a small font
or (more likely) superscript or subscript.
