how do i email a form using a hyperlink? word 2007

I created a question form to be filled out and emailed back to me. I want the
users to simply fill in the blanks, click a hyperlink and have the form
mailed back to me. Everytime I test it, I receive a blank email back.... Vey
simple form, no protection,
