Default Special Table of Contents request

I have Word 2010 and I know how to create a "regular" table of contents (TOC), but I need a special type. I need one that shows information kind of like an index would. For example, under a title in the table of contents, it normally shows the applicable page number. Well, normally if you have the same it will appear as many times as necessary in the TOC. title I need the title to appear ONCE and ALL the pages that go with it appear together (separated by commas) for that title.

I will try to show an example below of what I want:

Title............................................. .......4,17,38

INSTEAD OF THE NORMAL (shown below):
Title............................................. .......4
Title............................................. .......17
Title............................................. .......38
