Default Check Box Formatting

After inserting check boxes into a document, print and print preview does not
show the checkboxes. Instead the words {FORMCHECKBOX} appear. I've used
checkboxes many times in the past with no problem. Don't know why I'm getting
this problem now.

On the Print tab of Tools | Options, clear the box for "Field codes."
