Default Using WORD Fields to set the yes/no checkboxs based on user's answ

I have a form that requires me to ask the user a question that requires a yes
or no answer and based on that answer the appropriate checkbox is set to TRUE
(marked). Any ideas about the best way to accomplish this? HELP! :)

Default Using WORD Fields to set the yes/no checkboxs based on user's answ

If you need a dynamic (clickable) checkbox, there are a number of ways to
create and use clickable checkboxes in documents / templates. Some require
protected or locked forms, others do not. Take a look at the Checkbox
template available at:

for an exploration of these. Some utilize code, others do not.
