I want to put a survey together in Outlook. I have made my selections in
Options, and set it up. But, how in the body of the email do I add the radio
buttons, that is what is the setup to this?

This forum is for Word, however before you run over to the Outlook forum, I
should point out that as you have no control whatsoever of how your e-mails
are viewed by the recipient, you should consider sending a Word form as an
attachment, or those who will insist on viewing e-mails as text will not see
your check boxes/buttons.
