Default need a template letter from one Doctor to another MD re shared pat

My boss has asked me to draft a generic letter she can use when she has to
notify another Doctor of a shared patient... that is all the guidelines I've
got.
I've searched the web and can't seem to find any ideas or templates... Any
ideas out there?

Be a good employe and write the letter as requested, that's what you are

Please be nice and refer me to the proper place to post my question. I am
new at this job/position (one week). I have spent hours researching the web
and this afternoon spent a couple hours going through Microsoft templates for
ideas. If you don't have anything constructive to say then please keep your
impolite comments to yourself.

Then you either need to change your search terms or accept the fact that no
template exists on the web and create your own based on letters that your
doctor has received.

not to give advice on the *content* of documents written in Word. (This
purpose is not apparent if you are using the web interface provided by
Microsoft.) It is not the best place to ask your question, even if you get
some answers. Search the web for examples (using Google at
http://www.google.com/ or another search engine), or use your imagination.
