Default mail merge help

I need a step by step procedure for using a single datasource to merge to
multiple letters. I found what I need for word 2000 here

but I need this for word 2003. Any help would be great.

The procedure would be unchanged for Word 2003.

thanks, through trail and error I got it but I am having issue with
formatting. Do you have any recommendations on how to keep the formatting as
it in the template.

What sort of formatting problems?
