I thought I wanted to send the document in e-mail. Now I have the e-mail
'sending box information' on top of the page and I don't know how to remove
it. I decided I didn't want to send the document, but can't clear the 'form'
from the document page.

The e-mail header is a toggle setting, click the e-mail button (or file send
to mail recipient) button again.
