Default Need to create a fill in the blank template using existing form

HELP! I'm TRYING to convert a form of mine to a reusable TEMPLATE. However,
the help buttons in this program aren't much use. I don't know what I'm
doing wrong, but I'm sure it's easier than this.

I basically want to turn my form into a "fill-in-the-blank" type template so
I can re-use it again and again.

If someone out there knows what I'm talking about, please email me.
