Default help to set up survey

I need to set up a survey that I can email, they choose a choice or x a box
and emai it back to me.

right - now we might need a bit more information about what you would like us
to answer! Do you mean you want to put a box on the document? In which case
you could either use the drawing toolbar or use Insert--Symbol and locate a
shape!

If not, please be a bit more specific, you are more likely to geta reply!

Best bet is to read the tutorials (Dian Chapman's) on the following link.
Using tables to set up a protected form to collect responses can be
