Default formula to calculate form fill-in fields

I have created a lengthy survey in Word 2003. How do I set up a formula to
calculate form fill-in fields in a table that has subtotals and totals?

Use a formula field (=) to calculate the contents of the bookmarks created
by the form fields. Set the form field properties to calculate on exit.

e.g. {= ({REF Text1} + {REF Text2})}
