Default use mail merge to create telemarketing form, creates a new page f.

creating form using a database
need multiple records in a table on same page.
each record starts new page
any way to have a list like labels but w/ using tables.
its for a telemarketing list and creating fields for responses next to
preprinted names

one-to-many. You want the DATABASE field approach.
