Click Mailings Labels. In the Labels dialog, click the Options button. In
the Options dialog, set the Label Vendors dropdown to Microsoft (at the top
of the list, out of alphabetic order) and choose one of the two "1/4 Letter"
products. Click OK; back in the Labels dialog, click the New Document
button.

Sorry, I forgot that this one actually gives you only one of the four
"pages" on a quarter-size page.

Instead, choose "Avery US Letter" as the vendor and 8315 as the product
number. There are two of them, depending on whether you want a portrait or
landscape orientation.

That product number is not available in versions prior to 2007. If yours is
an older version, you can find templates for 1/4 page notes he

The product number Jay cited may not be available, but there are 1/4-page
Avery postcards and note cards available in earlier versions: 3263
(landscape) and 5845 (portrait).
