Default FILL IN THE BLANKS ON MS WORD ex. NAME__________

how do I make a field where responses can be made by other users without
messing up the spacing/formatting of the page?
Example:
last name:___________________
first name:____________________
employer:_______________________
phone number:___________________
etc.

What you are describing is a protected form. See
http://word.mvps.org/FAQs/Customizat...nTheBlanks.htm and especially
the forms tutorials by Dian Chapman that this article links to. In case the
tutorial doesn't make this clear, the usual way to preserve the size of form
fields is to put them in table cells of specified height and width (you can
give the cell a bottom border if you're really wedded to the idea of having
lines).
