I am building a TOC for a document with 11 Chapter headings and many paragraph
headings all on 1 level. The TOC generated only shows up to Chapter 5.1. Why?

I could make up some answers, but without knowing how you created the TOC,
they'd probably be wrong.

What do you mean by "all on 1 level"?
