Default Template vs Document

What is the relationship between a template and the document to which it is
attached? I will sometimes make format changes in a document. When I save the
document, Word (2003) does not always ask whether I want to make changes in
the template. Then when I open the document again, the format has reverted to
the template settings. Does this mean that every time I make a format change
in a document I'm working on that I need to go back and change the template
as well?

Note: I do not check the "add to template" box because I consider the style
is already part of the template, and I do not necessarily want to change it
permanently (i.e., single-spaced to double-spaced text). Second, I do not
check the "automatically update" box because I have been told (maybe in this
group) that it's not a good thing to do. Why I don't know, but that's my
recollection. Thanks.
