I don't know what I've pressed, but somehow the "symbol" under "insert" is
gone and have been replaced by "Number" and "Reference". How can I get
"Symbol" back?

Tools/Customize/Toolbars. In the list, select the "Menu bar" entry, then
click "Reset".
