To fix: right-click any toolbar. Click Customize. Select the Toolbars tab.
Select Menu Bar in the ToolBars list. Click Reset.

The standard toolbars are stored in the registry and it is quite common for
this part of the registry to suffer minor corruptions. If Jezebel's
suggested method doesn't fix it. See
