add a subject in the middle of an index
Select the appropriate piece of text in the document, and then press
Alt+Shift+X; this displays the Mark Index Entry dialog box with the selection displayed in the "Main entry" box. You can edit the text if you want to. Otherwise, just click the Mark button.
